Our contract with the State of Indiana expired December 31, 2006, so we no longer operate as a health plan for Hoosier Healthwise members in the State of Indiana. If you were sent a bill for services received prior to December 31, 2006, please call our Service Center at 1-866-930-0017 to leave a message and we will return your call within 48 hours. All other questions should be directed to your new health plan or to the Hoosier Healthwise Helpline at 1-800-889-9949.
